Severely atrophic maxilla with insufficient bone for standard implants. Four zygomatic implants placed bilaterally as anchors for full-arch fixed prosthesis. No bone graft required.
1. CBCT volumetric analysis and virtual zygomatic implant planning 2. Quad zygoma placement under GA 3. Immediate loading with PMMA provisional bridge 4. Final milled zirconia bridge at 6 months
All 4 zygomatic implants stable at 12 months. ISQ >70 on all fixtures. Patient eating full diet. Significant improvement in quality of life. No post-operative complications.
