Patient presented with persistent periapical pathology following two previous root canal treatments on upper left lateral incisor. Surgical endodontic intervention with retrograde MTA filling.
1. Full-thickness mucoperiosteal flap 2. Apical resection 3mm 3. Retrograde cavity preparation with ultrasonic tip 4. MTA retrograde fill 5. Primary closure
Uneventful healing at 3 months. Complete periapical healing confirmed radiographically at 12 months. Tooth fully functional.
